Individualized Service Plan: Each new resident arrives with self-knowledge, challenges, desires and possibilities. Our case management is styled as an Individualized Service Plan. It follows a collaborative process: assessment; planning; facilitation; care coordination; evaluation; and advocacy for options, activities and services—with the goal of meeting an individual’s comprehensive needs through communication and available resources.
